We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policies.
Consultations, Radiographs and Periodontal Maintenances (cleanings) are collected in full on the date of service unless you are insured by Delta Dental, Cigna Dental or Aetna Dental. For surgical procedures, our office would be happy to submit a pre-treatment estimate to your insurance company after your initial exam. We will collect the patient portion of the pre-treatment estimate or 60 percent of the fee on the date of service for surgical procedures.
We Are a Premier Provider for Delta Dental, Cigna Dental and Aetna Dental.
Your insurance is an agreement between you and your insurance company. You are ultimately responsible for your account balance.
Outstanding insurance claims over 45 days will be reassigned to you, regardless of the reason for the delay the insurance company gives.
In the event that you account is past due, it will be turned over to Midwest Credit & Collection, Inc. of Decatur, IL. In this event, you will be responsible for all reasonable fees necessary for the collection of the delinquent account including, but not limited to, collection agency fees up to 1/2 of the appointment fee and non-sufficient funds penalties on returned checks.
Payment in full for all services is due on the date of service.
