We are here to provide you with periodontal care while protecting you, ourselves, and the community.
Dr. Maggie Pedraza has successfully completed the Coronavirus Infection Control for Dentistry course.
If you have a true dental emergency, please leave a message at Forsyth IL Periodontal Office Phone Number 217-876-0547 and we will return your call. As our current phone greeting states, messages are being checked at 9:00 AM, Noon, and 3:00 PM. Please leave your name, your phone number and your pharmacy name.
In light of the Coronavirus outbreak, all patients will need to complete this questionnaire to help assure your safety and the safety of those around you before being seen at the office. Thank you in advance for completing it.

If all your answers are NO, we will see you at your regularly scheduled appointment time, where these questions may be asked again and your temperature taken with a no touch thermometer. We will schedule your appointment as promptly as possible. Please allow approximately (60) minutes for your initial visit.
Every attempt will be made to see you as soon as possible.
The nature of our practice is to give our patients the utmost in care and service while staying on schedule. However, occasional delays happen, including emergencies, so please remember we will give you the same careful attention during your appointment.
All surgery appointments require a 2-day notice of cancellation or rescheduling in order to avoid a cancellation fee. All other appointments require a 24-hour notice.
